BREATHE - Bringing Resources to Eliminate All Tobacco Through Health Education
In partnership with the University of Ottawa Heart Institute, SANSAR has launched a new free counseling program using the Ottawa Model for Smoking Cessation. This program pairs a counselor with each patient who is interested in or would like to find out more information about quitting. Together, they work on a personalized quit plan. Patients are connected to tools and resources to help them reach their individual goals.
How it works:
Patients are referred to the program by their physicians.
Patients meet a counsellor, who will learn more about the patients and their needs.
The counsellor and the patient create a plan together based on various options available to them.
Patients begin their personalized plan, and receive coaching and support along the day.
The counsellor provides resources, tools and guidance to help patients achieve their goals.
Regular updates about the patient progress can be sent to their family physicians.
